2. Nature and Scope of Services
My Health File provides:

Digital organization and structuring of medical records

Preventive annual health review services

Compilation and structured presentation of existing medication history

Structured medical opinion summaries based on provided documentation

Coordination of diagnostic testing through accredited third-party laboratories

Optional in-person home consultation services
All medical services are governed exclusively by the laws of India.
Services are provided only by Registered Medical Practitioners holding valid Indian medical licenses.
The Services are preventive and advisory in nature and are not a substitute for emergency care, hospitalization, or continuous management under a treating physician.
3. No Emergency Services
The Company does not provide emergency medical services.
If you experience a medical emergency, you must immediately contact:

The nearest hospital
The Company shall not be responsible for any delay in seeking emergency care.
4. User Eligibility
By using the Services, you confirm that:
The Company may request proof of authorization where required.
5. User Responsibilities
You agree that:

All medical information, reports, and disclosures provided are accurate and complete.

You will not intentionally conceal relevant medical history.

Medical opinions are based solely on the information provided.

You will seek direct medical care when advised or when necessary.
The Company is not liable for any consequence arising from inaccurate, incomplete, or misleading information provided by the User.
6. Medical Opinion Disclaimer
Medical opinion summaries:

Are based on documentation shared by the User

Are preventive and advisory in nature

Do not constitute hospital discharge summaries

Do not replace specialist evaluation or in-person clinical examination unless explicitly stated
Medical decisions remain the responsibility of the patient and their treating physician.
The Company does not guarantee medical outcomes.
7. Laboratory Services
Diagnostic testing is conducted by independent, accredited third-party laboratories.
The Company:

Does not operate its own laboratory

Does not alter laboratory findings

Is not responsible for laboratory operational errors
Any disputes regarding sample collection, accuracy of testing, delays, or laboratory conduct must be addressed directly with the respective laboratory.
Laboratory cancellation or refund policies are governed by the partner laboratory.
8. In-Person Home Consultation
Where home consultation services are requested:

Prior informed consent must be provided

The User must ensure a safe and appropriate environment

The Company reserves the right to decline visits deemed unsafe
Documentation generated during home consultation becomes part of the patient’s medical record.
9. Payments
Payments may be processed via:
Payment by a third party (including NRIs) does not grant medical decision-making authority unless expressly authorized by the patient.
All fees are displayed prior to confirmation.
10. Refund Policy
All payments for preventive health review and consultation services are non-refundable once the service process has commenced.
Service commencement includes but is not limited to:
In exceptional circumstances where services have not commenced, refund requests may be evaluated at the sole discretion of the Company.
Laboratory-related payments are subject to the respective partner laboratory’s cancellation and refund policies.
11. Limitation of Liability
To the fullest extent permitted under applicable Indian law:
The Company shall not be liable for:

Indirect, incidental, or consequential damages

Medical outcomes beyond advisory scope

Emergency events

Laboratory operational errors

Decisions taken solely by the patient without consulting treating physicians
The Company’s total liability, if any, shall not exceed the amount paid for the specific service giving rise to the claim.
12. Indemnification
You agree to indemnify and hold harmless Dr Mads Technologies Pvt Ltd and its practitioners from any claims, liabilities, damages, or legal costs arising from:

False or incomplete information provided by you

Misuse of Services

Failure to follow medical guidance

Unauthorized access due to your negligence
13. Record Retention
Medical records may be retained for the minimum period required under applicable Indian medical and legal regulations.
Account deletion requests may not result in immediate deletion of records required to be retained by law.
14. Intellectual Property
All platform content, structure, documentation format, and materials are the intellectual property of Dr Mads Technologies Pvt Ltd.
Unauthorized reproduction, copying, or distribution is prohibited.
15. Privacy
Use of the Services is also governed by the Company’s Privacy Policy.
By using the Services, you consent to the collection and processing of sensitive personal data as described therein.

These Terms & Conditions shall be governed exclusively by the laws of India.
All disputes shall be subject to the exclusive jurisdiction of the courts located in Hyderabad, Telangana, India.

Continued use of the Services after updates constitutes acceptance of the revised Terms.
